| ARE YOUR KIDS READY FOR 7TH GRADE?
REMINDER: Families of 6th Grade Students:
Minnesota state law and school policy requires students entering 7th grade to have the following shots or legal exemptions completed to attend school.
MMR: (measles, mumps and rubella): Two doses given after the first birthday.
Td/Tdap: (tetanus, diphtheria and pertussis); One dose given after 11th birthday.
Hepatitis B: A series of three shots given over a six-month period.
Varicella: Two doses given after the first birthday, or date of disease.
Polio: At least three doses.

Sports Physical Forms:
A sports physical must be on file in the health office in order for your child to participate in athletics.

Medication Administration Policy:
Written authorization from both parent/guardian AND physician/licensed prescriber must be received before ANY medication can be administered. It needs to be renewed annually or whenever the medication changes.
